About This Plate
Disability Special Group
This plate is issued to individuals who qualify for disabled person’s plates for use in specially designated handicapped parking spaces. This plate may be ordered with the Life Elevated Skier, Life Elevated Arches, or plain white background, and may be personalized with up to five alphanumeric characters, or up to four for a motorcycle.
Requirements
- Completed Disabled Person and Physician Certification (Form TC-842), signed by a licensed physician, physician assistant, physical therapist or nurse practitioner.
